Testimonial

I've been on a diet my whole life, but this is the first time I do a nutrition program where I feel relaxed, having fun and enjoying eating new foods. Thank you!

Corporate Health Survey

The following survey will help you assess the strengths and improvement areas in both your company’s wellness programs and your overall company culture. As you go through the survey you will be able to identify key tactics that might be missing from your strategy.

Do keep in mind though, that a strategy does not come together by working backwards. If you don’t have a strategy already, this survey will help you assess your starting point to begin to create a robust one.

Effectiveness of Your Wellness Strategy

Y N
1 Do you have a wellness strategy, a plan and timeline for your wellness initiatives?
2 Do you have a well articulated wellness communication plan that ties into your company’s overall communications?
3 Do you have programs in five key areas of a wellness strategy:  Awareness, Education, Behavior Change, Support and Motivation?

The quality and effectiveness of your wellness initiatives is highly dependent on how well you plan them from the start.  Hap-hazard activities here and there will lead to hap-hazard results.

Culture of Wellness & Prevention

Y N
4 Does your company offer annual general health screenings?
5 Do your company benefits encourage preventive care either through full coverage of preventive services or other incentives?
6 Does your company sponsor onsite health programs (nutrition counseling, physical trainers, such as nurses, EAPs, etc.)?
7 Does your company sponsor wellness fairs?
8 Does your company have a non-smoking policy?
9 Does your benefits package encourage smoking cessation?

Depending on the size of your company and your budget, these are some of the corner stones of your wellness initiative. They encourage: 1) early detection of potential risks, 2) healthy behaviors to maintain low risk people low, and bring medium-high risk people down, 3) general awareness of health.

Through our strategy model you can identify step by step which of these initiatives is right for your company.

Culture of Life Balance

Y N
10 Does your company offer stress-management programs?
11 Do the leaders of your company role-model work–life balance?
12 Does your company culture support employees striving for full career satisfaction?
13 Does your company offer flexible working arrangements and/or flexible working hours?

Stress is one of the top drivers of reduced productivity and is a risk factor for a variety of health conditions. Your company’s culture around work ethics and life-balance can have a significant impact in how your employees cope with life stressors.

Culture of Exercise

Y N
14 Does your company have an onsite gym or fitness facility?
15 If so, do you believe the gym/fitness equipment is getting enough use?
16 Does your company offer reimbursement for gym memberships?
17 Has your company sponsored charity walks/runs or other similar activities?
18 Has your company ever conducted fitness contests or programs to stimulate physical activity in your employees?
19 Does your workplace have showers or locker rooms onsite?
20 Does a typical workday involve physical activity for a majority of your company’s employees (anything from walking to heavy labor)?
21 Does your company offer flexibility of schedules to foster exercise?
22 Does your company distribute messages that support and encourage physical activity?

Low physical activity is one of the main risk factors for a variety of health conditions. The good news is that encouraging exercise can be one of the simplest tactics to implement. The bad news is that getting your employees to truly become more active can be tricky. These initiatives must be well planned and executed so that you don’t create ‘noise’ around wellness and lose people’s interest.

Culture of Healthy Eating

Y N
23 Do you have vending machines onsite?
24 Do you have access to a cafeteria, catering services or vending cart?
25 If so, do they offer healthy options on a daily basis?
26 Does your food provider disclose nutrition information of their food choices?
27 Does your company offer kitchen/pantry space (refrigerators, microwaves, tables, sink, etc.)?
28 Do employees have a room or space away from their workstations where they can eat their meals?
29 Does your company distribute information that encourages healthy eating?
30 Does your company typically provide healthy foods and snacks at meetings?

However your company approaches food, whether it is in the snacks you offer at meetings, the restaurants you pick for off-site gatherings, or the presence of vending machines, has an important effect in the success or failure of your wellness initiatives. Your employees spend the majority of their time at work so the environment can either be supportive of their efforts or detractive.

Culture of Personal Empowerment

Y N
31 Is teamwork one of your company strengths?
32 Do your employees take full ownership and accountability for the quality of their work?
33 Is the morale and engagement of your employees high?
34 Are your employees inspired to come to work?
35 Do your employees take the initiative to suggest process/procedure improvements?
36 Do your employees participate in problem-solving?
37 Do your employees feel free to express their opinions?

Whether your company has a strong or weak culture of personal empowerment has a huge effect not only in your employees’ sense of accountability towards their own work and their health, but also on the quality of business results you can obtain from your team. When teams and organizations know how to create strong individual leadership it reflects in the quality of the work environment, in the quality of teamwork and ultimately in their business results.

When you submit this survey to us we will provide you with a complimentary consultation on the strengths and improvement needs in your wellness strategy. Through the consultation we will help you determine where to start.
